CCM Member Towns Eligible for ProAct Prescription Discount Card
IMPORTANT INFORMATION FOR RESIDENTS OF MIDDLEBURY AND WATERBURY
Residents of the member towns of the Connecticut Conference of Municipalities (CCM) are able to receive a FREE PRO ACT PRESCRIPTION DISCOUNT CARD which reportedly can save an average of 45 percent off the retail price of medications, with discounts also available for vision, LASIK and hearing care services. (This benefit is especially valuable for people without health insurance or a traditional pharmacy benefit plan, or for patients with prescriptions not covered by insurance.)
Here are some of the specifics of this program:
- Coverage is available for all family members, EVEN PETS, as long as it’s a medication that also treats a human condition
- Cards can be mailed to the resident, or downloaded and printed
- Cards are automatically activated and widely accepted at all national chain pharmacies and most local independent pharmacies
- Vision discounts are 20% off the usual/customary fee for eye exams; 50% off frames/lenses; everyday low pricing and free shipping on online lenses
- LASIK discounts are 40-50% off the national average cost (accepted at 600+ locations nationwide from credentialed physicians)
- Hearing services discounts are approximately 35-65% savings from a national network of local ear physicians and audiologists and on brand-name hearing aids/products
